摘要:目的:探讨茵陈蒿汤调控Janus激酶2(JAK2)/信号转导和转录激活因子3(STAT3)通路对乙型病毒性肝炎(HB)大鼠肝功能和免疫功能的影响。方法:通过尾静脉注射乙型肝炎病毒(HBV)构建HB大鼠模型(50只),分为模型组、茵陈蒿汤低剂量组、茵陈蒿汤高剂量组和茵陈蒿汤高剂量+JAK2激活剂(Colivelin)组,每组10只,另取10只大鼠注射生理盐水作为对照组。酶联免疫吸附试验(ELISA)检测血清肝功能指标和炎症指标,流式细胞术检测血液淋巴细胞中T淋巴细胞17(Th17)和调节T细胞(Treg)亚群占比,HE和Masson染色检测肝组织病理损伤,免疫组化检测肝组织中转化生长因子-β1(TGF-β1)、纤连蛋白(FN)、肌动蛋白α(α-SMA)蛋白表达,Western blotting检测肝组织中半胱天冬酶-3(Caspase-3)、B淋巴细胞瘤-2相关X蛋白(Bax)及JAK2/STAT3通路相关蛋白表达。结果:与对照组比较,模型组大鼠肝组织细胞形态肿胀,肝小叶结构被破坏,可见炎性浸润和胶原纤维,肝组织中TGF-β1、FN、α-SMA蛋白阳性表达增多,丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、白介素-4(IL-4)水平,Th17细胞占比,Th17/Treg、p-JAK2/JAK2、p-STAT3/STAT3值以及Caspase-3、Bax蛋白表达水平均升高,γ干扰素(IFN-γ)、IL-2水平,Treg细胞占比降低(P<0.05);与模型组比较,茵陈蒿汤低、高剂量组大鼠肝组织细胞形态恢复,炎性浸润和胶原纤维减少,肝组织中TGF-β1、FN、α-SMA蛋白阳性表达减少,ALT、AST、IL-4水平,Th17细胞占比,Th17/Treg、p-JAK2/JAK2、p-STAT3/STAT3值以及Caspase-3、Bax蛋白表达水平均降低,IFN-γ、IL-2水平,Treg细胞占比升高,且茵陈蒿汤高剂量组指标变化较茵陈蒿汤低剂量组显著(P<0.05);与茵陈蒿汤高剂量组比较,茵陈蒿汤高剂量+Colivelin组肝小叶结构模糊,炎性浸润和胶原纤维增多,肝组织中TGF-β1、FN、α-SMA蛋白阳性表达增多,ALT、AST、IL-4水平,Th17细胞占比,Th17/Treg、p-JAK2/JAK2、p-STAT3/STAT3值以及Caspase-3、Bax蛋白表达水平均升高,IFN-γ、IL-2水平,Treg细胞占比降低(P<0.05)。结论:茵陈蒿汤可能通过调控JAK2/STAT3通路缓解HB大鼠肝功能和免疫功能损伤。

Abstract:
Objective: To investigate the effects of Yinchenhao Decoction on liver function and immune function in rats with hepatitis B (HB) by regulating the Janus kinase 2 (JAK2)/signal transducer and activator of transcription 3 (STAT3) pathway. Methods: An HB rat model was established by injecting hepatitis B virus (HBV) into the tail vein. The modeled rats were divided into model group, low-dose Yinchenhao Decoction group, high-dose Yinchenhao Decoction group, and high-dose Yinchenhao Decoction+JAK2 activator (Colivelin) group, with 10 rats in each group. Another 10 rats injected with normal saline served as the control group. Serum liver function indicators and inflammatory markers were measured using enzyme-linked immunoadsordent assay (ELISA). The proportions of T helper 17 (Th17) and regulatory T (Treg) cell subsets in blood lymphocytes were determined by flow cytometry. Pathological damage in liver tissue was assessed using HE and Masson staining. The protein expression of transforming growth factor-β1 (TGF-β1), fibronectin (FN), and α-smooth muscle actin (α-SMA) in liver tissue was evaluated by immunohistochemistry. Western blotting was used to detect the expression of Caspase-3, Bax, and JAK2/STAT3 pathway-related proteins in liver tissue. Results: Compared with the control group, rats in the model group showed swollen hepatocytes, disrupted hepatic lobule structure, visible inflammatory infiltration and collagen fibers, along with increased positive expression of TGF-β1, FN, and α-SMA proteins in liver tissue. Levels of ALT, AST, IL-4, the proportion of Th17 cells, the ratios of Th17/Treg, p-JAK2/JAK2, and p-STAT3/STAT3, as well as the expression levels of Caspase-3 and Bax proteins were significantly elevated, while levels of IFN-γ, IL-2, and the proportion of Treg cells were significantly reduced (P<0.05). Compared with the model group, the low-dose Yinchenhao Decoction group and high-dose Yinchenhao Decoction group exhibited restored hepatocyte morphology, reduced inflammatory infiltration and collagen fibers, and decreased positive expression of TGF-β1, FN, and α-SMA in liver tissue. Levels of ALT, AST, IL-4, the proportion of Th17 cells, the ratios of Th17/Treg, p-JAK2/JAK2, and p-STAT3/STAT3, as well as the expression of Caspase-3 and Bax were significantly decreased, while levels of IFN-γ, IL-2, and the proportion of Treg cells were significantly increased (P<0.05). These effects were more pronounced in the high-dose Yinchenhao Decoction group than in the low-dose Yinchenhao Decoction group (P<0.05). Compared with the high-dose Yinchenhao Decoction group, the high-dose Yinchenhao Decoction+Colivelin group showed blurred hepatic lobule structure, increased inflammatory infiltration and collagen fibers, and enhanced positive expression of TGF-β1, FN, and α-SMA in liver tissue. Levels of ALT, AST, IL-4, the proportion of Th17 cells, the ratios of Th17/Treg, p-JAK2/JAK2, and p-STAT3/STAT3, as well as the expression of Caspase-3 and Bax were significantly elevated, while levels of IFN-γ, IL-2, and the proportion of Treg cells were significantly reduced (P<0.05). Conclusion: Yinchenhao Decoction may alleviate liver function and immune function impairment in HB rats by regulating the JAK2/STAT3 pathway.
