腕部改良术式针刀联合火龙罐治疗腕管综合征的疗效观察*

摘要：目的：探讨中医技术火龙罐在针刀治疗腕管综合征（CTS）患者中的应用效果。方法：76例患者随机分为对照组（38例）和治疗组（38例），对照组采用腕部改良术式针刀治疗及常规护理，治疗组在对照组的基础上加用中医技术火龙罐疗法，比较两组治疗效果。分别于治疗前后对CTS症状严重程度量表（SSS）积分、疗效指数，以及正中神经感觉传导速度（SCV）、感觉动作电位波幅（SNAP）、末梢运动潜伏期（DML）进行评定。结果：治疗后治疗组的愈显率（痊愈+显效）及总有效率均高于对照组（P<0.05）。两组患者治疗后SSS积分均较治疗前降低（P<0.05），且治疗组患者治疗后SSS积分低于对照组（P<0.05）。两组患者治疗后SCV、SNAP均较治疗前升高（P<0.05），DML较治疗前降低（P<0.05），且治疗组患者治疗后SCV、SNAP均高于对照组（P<0.05），DML低于对照组（P<0.05）。结论：中医技术火龙罐应用于针刀治疗腕管综合征中疗效更显著，能更好促进患者康复。

Abstract：

Objective: To evaluate the therapeutic effect of Huolong Cupping (火龙罐), a traditional Chinese medicine (TCM) technique, combined with modified acupotomy in patients with carpal tunnel syndrome (CTS). Methods: Seventy-six CTS patients were randomly divided into a control group (38 cases) and a treatment group (38 cases). The control group received modified acupotomy and routine care, while the treatment group underwent additional Huolong Cupping therapy. Clinical outcomes were assessed using the symptom severity scale (SSS), efficacy indices, and electrophysiological parameters, including sensory conduction velocity (SCV), sensory nerve action potential amplitude (SNAP), and distal motor latency (DML) of the median nerve before and after treatment. Results: After treatment, the marked effectiveness rate (cure + significant improvement) and total effectiveness rate in the treatment group were significantly higher than those in the control group (P<0.05). Both groups showed reduced SSS scores post-treatment (P<0.05), with lower scores in the treatment group compared to the control group (P<0.05). SCV and SNAP increased, while DML decreased in both groups after treatment (P<0.05). Furthermore, the treatment group exhibited higher SCV and SNAP values and lower DML values than the control group (P<0.05). Conclusion: The integration of Huolong Cupping with modified acupotomy demonstrates superior efficacy in treating CTS, significantly enhancing patient recovery.
