摘要:总结肠道菌群通过微生物--脑(MGB)轴影响缺血性脑卒中发生发展的机制,包括肠道菌群代谢产物调节卒中过程、释放神经递质影响大脑功能、通过免疫炎症反应参与卒中进程。阐述针刺通过调控肠道菌群结构、改善代谢产物平衡、调节神经递质分泌及抑制过度免疫炎症反应等途径,对MGB轴功能的调节作用及研究进展。分析中医辨证施治下不同证型缺血性脑卒中的针刺选穴及菌群调节特点。旨在为揭示针刺治疗缺血性脑卒中的科学内涵提供理论依据,并为临床优化治疗方案、发挥中医特色优势提供新方向。

Abstract:This review summarizes the mechanisms by which gut microbiota influences the occurrence and development of ischemic stroke through the microbiota-gut-brain (MGB) axis, including the regulation of stroke processes by gut microbiota metabolites, the impact on brain function through the release of neurotransmitters, and participation in stroke progression via immune-inflammatory responses. It elaborates on the regulatory effects and research progress of acupuncture on MGB axis function by modulating the structure of gut microbiota, improving the balance of metabolites, regulating neurotransmitter secretion, and inhibiting excessive immune-inflammatory responses. The analysis focuses on the selection of acupuncture points and the characteristics of microbiota regulation in different syndromes of ischemic stroke under the guidance of TCM syndrome differentiation and treatment. The aim is to provide a theoretical basis for revealing the scientific connotation of acupuncture in treating ischemic stroke and to offer new directions for optimizing clinical treatment plans and leveraging the unique advantages of TCM.
