补阳还五汤加减治疗气血瘀阻型小儿腺样体肥大的疗效观察*

摘要：

目的：观察补阳还五汤加减治疗气血瘀阻型小儿腺样体肥大的临床疗效。方法：将84例气血瘀阻型腺样体肥大患儿随机分为治疗组和对照组，每组各42例。对照组患儿予糠酸莫米松鼻喷雾剂和孟鲁司特钠咀嚼片治疗，治疗组患儿予补阳还五汤加减治疗，连续治疗8周。比较两组疗效、临床症状（主症及次症）评分、腺样体体积评分及儿童阻塞性睡眠呼吸暂停生活质量量表（OSA-18）评分。结果：两组各脱落2例，最终两组各纳入40例。总有效率对照组为80.00%（32/40），治疗组为95.00%（38/40），治疗组高于对照组（P<0.05）；治疗4周、8周两组患儿主症、次症、腺样体体积及OSA-18评分均较治疗前降低（P<0.05）；治疗8周后治疗组患儿次症、腺样体体积和OSA-18评分均低于对照组（P<0.05）。结论：补阳还五汤加减治疗气血瘀阻型小儿腺样体肥大具有良好的临床疗效，能改善患儿临床症状、缩小腺样体体积并提高生活质量。

Abstract：

Objective: To observe the clinical efficacy of modified Buyang Huanwu Decoction in treating pediatric adenoid hypertrophy with the pattern of qi and blood stasis. Methods: Eighty-four children with adenoid hypertrophy of the qi and blood stasis pattern were randomly divided into a treatment group and a control group, with 42 cases in each group. The control group received mometasone furoate nasal spray and montelukast sodium chewable tablets, while the treatment group was treated with modified Buyang Huanwu Decoction. The treatment course for both groups was 8 consecutive weeks. The therapeutic efficacy, clinical symptom scores (main and secondary symptoms), adenoid volume score, and the Obstructive Sleep Apnea-18 Quality of Life (OSA-18) score were compared between the two groups. Results: Two cases dropped out from each group, resulting in 40 cases being included in the final analysis for each group. The total effective rate was 80.00% (32/40) in the control group and 95.00% (38/40) in the treatment group, with the treatment group showing a significantly higher total effective rate than the control group (P<0.05). After 4 and 8 weeks of treatment, the scores for main symptoms, secondary symptoms, adenoid volume, and OSA-18 in both groups decreased compared to those before treatment (P<0.05). After 8 weeks of treatment, the scores for secondary symptoms, adenoid volume, and OSA-18 in the treatment group were lower than those in the control group (P<0.05). Conclusion: Modified Buyang Huanwu Decoction shows good clinical efficacy in treating pediatric adenoid hypertrophy with the qi and blood stasis pattern. It can improve clinical symptoms, reduce adenoid volume, and enhance quality of life.
