七味疗疮解毒散穴位贴敷联合富血小板血浆凝胶治疗住院感染性压疮的临床研究*

摘要：

目的：探讨七味疗疮解毒散穴位贴敷联合富血小板血浆（PRP）凝胶治疗住院感染性压疮的临床疗效。方法：纳入90例存在感染性压疮患者（Ⅲ~Ⅳ期），采用随机数字表法分为观察组、对照1组和对照2组，每组各30例，所有患者均予以美宝湿润烧伤膏，对照1组在此基础上予以PRP凝胶治疗，对照2组在此基础上予以七味疗疮解毒散穴位贴敷，观察组在此基础上予以七味疗疮解毒散穴位贴敷联合PRP凝胶治疗。对3组患者临床疗效进行评价并对比，对3组完全愈合患者的创面愈合时间及创面愈合质量进行比较。记录3组患者创面细菌培养转阴时间及不同时间点（治疗1个月、3个月后）创面愈合情况。于治疗前后，分别抽取患者的静脉血检测患者血清C-反应蛋白（CPR）、红细胞沉降率（ESR）及降钙原素（PCT）水平。观察3组患者不良反应发生情况。结果：观察组总有效率为93.33%（28/30），高于对照1组的73.33%（22/30）和对照2组的70.00%（21/30）（P＜0.05）；与对照1组和对照2组比较，观察组完全愈合患者的创面愈合时间明显缩短，温哥华瘢痕量表（VSS）评分均降低（P<0.05）。观察组患者创面转阴时间在＜2周的比例高于对照1组和对照2组（P<0.05）；观察组患者治疗1个月和3个月后的压疮愈合计量表（PUSH）评分均低于对照1组和对照2组（P<0.05）。治疗3个月后，观察组患者CRP、ESR和PCT水平均低于对照1组和对照2组（P<0.05）。3组总不良反应发生率比较，差异无统计学意义（P>0.05）。结论：七味疗疮解毒散穴位贴敷联合PRP凝胶可有效抑制住院感染性压疮创面炎症发展，缩短创面转阴时间，加快压疮创面修复，且具备安全性。

Abstract：Objective: To evaluate the clinical efficacy of acupoint application of Qiwei Liaochuang Jiedu Powder (a seven-herb formula for ulcer detoxification) combined with platelet-rich plasma (PRP) gel in treating hospitalized patients with infected pressure ulcers. Methods: Ninety patients with stage Ⅲ-Ⅳ infected pressure ulcers were randomly divided into an observation group, Control group 1, and Control group 2 (30 cases each). All groups received MEBO Moist Exposed Burn Ointment. Control group 1 additionally received PRP gel, Control group 2 received acupoint application of Qiwei Liaochuang Jiedu Powder, and the observation group received both interventions. Clinical efficacy, wound healing time, Vancouver Scar Scale (VSS) scores, bacterial clearance time, Pressure Ulcer Scale for Healing (PUSH) scores at 1 and 3 months, and serum levels of C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and procalcitonin (PCT) were compared. Adverse reactions were recorded. Results: The observation group showed a total effective rate of 93.33% (28/30), significantly higher than 73.33% (22/30) in Control group 1 and 70.00% (21/30) in Control group 2 (P<0.05). The observation group exhibited shorter wound healing time, lower VSS scores (P<0.05), and higher rates of bacterial clearance within <2 weeks compared to both control groups (P<0.05). PUSH scores at 1 and 3 months, as well as CRP, ESR, and PCT levels at 3 months, were significantly lower in the observation group (P<0.05). No significant difference in adverse reactions was observed among groups (P>0.05). Conclusion: The combination of Qiwei Liaochuang Jiedu Powder acupoint application and PRP gel effectively suppresses inflammatory progression, shortens bacterial clearance time, accelerates wound repair in infected pressure ulcers, and demonstrates favorable safety.
